For planned admission, patients and their companion/s are directed to the Admitting Section located at the lobby of the main hospital building.
Before admission, our Admitting staff will request for the following:
· Doctor’s Order for Admission indicating directives regarding the patient’s case
· Accredited HMO-issued member’s ID
· Philhealth ID
· Persons with Disability or Senior Citizen ID
· Letter of Guarantee or Letter of Authorization (LOA) from the company or HMO Coordinator
· Any government-issued ID with a photo
· RT-PCR or Rapid Antigen is required if the patient is symptomatic with Covid-related symptoms*
Note: *During a pandemic or such unusual situation, health protocols mandated by the Philippine Department of Health and guidelines issued by the CSMC Infectious Medicine Unit will be strictly implemented.
Please also observe the following guidelines upon admission:
1. Admitting may start accepting patients as early as 8:00 am for planned or direct admission and the cut-off time for discharge including bill settlement is on or before 12 noon. Anytime you get admitted today is considered as one (1) day until the following day at 12:00nn. Additional room charge (½ day rate if the patient will be discharged between 12:01 pm – 6:00 pm, and full day rate after 6:01 pm) will be added to the hospital bill if the patient wishes to stay in the room beyond the cut-off time.
2. Maximum of two (2) companions are allowed per patient in the clean ward (private rooms) and one (1) companion for semi-private and service wards. Companion must be asymptomatic (no Covid-related symptoms). It is also recommended that the companion should be less than 60 years old and without co-morbidities such as hypertension and/or diabetes.
3. No companions are allowed in the Covid wards and ICU (Covid ICU and Clean ICU).
